Disease Management

One of the most challenging issues in the management of chronic health conditions is provider compliance with current clinical guidelines and patients’ adherence with their treatment plans. To address these issues the Home Automated Telemanagement (HAT) system has been developed. The HAT system is aimed to facilitate management of chronic diseases according to current clinical guidelines. HAT is designed to support a multi-disciplinary approach in patient self-management which includes regular patient assessment, disease-specific education, control of patient compliance with treatment plans, implementation of health behavior change models and social support. The HAT system provides computerized decision support and alert mechanism to facilitate clinical guidelines implementation. As part of Asthma HAT, we developed a web-based decision support for clinicians to facilitate implementation of NAEPP guidelines for asthma management. The key feature of this project is implementation of institution-wide system providing help in generation of asthma action plans based on national guidelines and adopted by local leaders in asthma care for use in primary and specialty care clinics.
